I'm wondering why Kareha has thread titles on top of the page and Wakaba doesn't. Forgive my ignorance but is there any special reason for this?

If you want an imageboard with a top index, just use kareha in text-mode with images on.
Are you talking about the title in the window's titlebar?
Because Wakaba is http://www.2chan.net/ and Kareha is http://www.2ch.net/.

Yes. At least I can't discuss seriously in an imageboard.

Titles don't matter on imageboards since theyre not about text/discussion.
That's why there are discussion boards.
See? Image. Discussion. One's (theoretically) about images with brief stupid comments, one's for serios discussion where thread titles make any difference.
The equivalent I think would be Catalog mode for an imageboard
